Established patients may schedule online.  Please visit the PATIENT PORTAL tab. This will take you to our electronic health records patient portal.  Please note:  In order to gain access to our patient portal for the first time, you will need to contact our office and give us your e-mail address so that we can send you a link to sign up. If you already signed up for the patient portal, please sign in directly and follow the prompts.

New patients please call our front desk at 360 878 8735 to schedule. This is to ensure we are a good fit for you, send you the appropriate forms and collect your insurance information. 

 No insurance?  No problem.  We have cash services and consider our membership for discounted visits.  Call for information on membership here 360-878-8735.
